Check for nipple blebs, (clogged ducts).  Due to her returing to work, maybe 
the breasts are not being emptied enough.  Have mom look at nipples 
immediately post feed.  Look for any white looking pimple type things.  This 
has happened to a few of my moms and is VERY painful until they get the 
blockage cleared.
