I use a HICFA form, which is a widely available standard form (I buy them at
Staples, w/1 carbonless copy), and I fill out the "Provider" section, using
standard codes (e-mail me for the codes, or maybe someone else will post
them), but I have my clients pay me up front and submit the form on their
own. In the packet of stuff I give them, I include simple instructions for
how to submit it to ins. - get a written referral from MD office, keep
copies of everything you send, and a letter from me to the insurance company
explaining *why* it's so important for them to cover LC services.

This way I get paid, and I'm *helping* them file but not taking ownership of
that process. Works fine for me; some companies pay, some don't. I'm not
willing to take on the hassles of billing individual companies - *certainly*
not for the very modest fee that I charge!

I agree that there needs to be coverage for our services, but my skills are
in helping women BF, not in doing insurance company paperwork. I used to
feel guilty about this minimalist attitude towards the ins. issue, till I
finally "got it" how very much better-off financially most of my clients are
than I am; it really hit me hard that most of these clients don't even bat
an eyelash at my fee. They pay more than that to get their nails done or
their floors vacuumed! And for those who meet WIC income guidelines, I have
a significantly reduced fee.
