Cathy I had a mother-baby that was a bit similar.  Baby also had
ankyloglossia, though at the time I saw this mother it had just torn and
the tongue was now free.  Baby was a very unhappy little one, though
nutritionally looking OK.  Mother was totally stressed and not enjoying
mothering at all.  We tried various techniques at the consult without much
improvement, then I suggested mother express and we see how bub goes with
finger feeding.  She used an Avent Isis hand pump, pumped twice and had
about 40 or 50 mls (nearly 2 ounces) of milk in the bottle in no time at
all.  Most incredible rapid milk ejection I've ever seen.
For the next two days I asked her to pump before each feed until that first
ejection was finished, then latch baby.  She came back to see me on the
second day and I didn't recognise the baby - she was a completely different
child, and the parents were all smiles.

Another severe breast refusal problem I've seen turned out .. only in
hindsight, not picked up at the time ... to be severe allergy to something
the mother was consuming.  In this case it was garlic and horseradish
capsules for sinusitis.  First time toddler consumed garlic in his solid
diet he had an anaphylactic reaction.
