Pam asks about pumping/expressing "uses" where pumps are not readily available.

Most public hospitals and even private clinics here in Argentina do not
have pumps available. I do not know how milk extraction is handled in the
few Babyfriendly Hospitals we have, but I can tell you how it is done here
in Bariloche:

Many mothers use hand extraction or use a manual milk extractor (we call
"syringe-type", I hope you understand which one I mean). I have seen
mothers of preemies maintaining an excellent supply with these seemingly
inadequate milk extractors. The new Medela hand pump (lower priced than the
previous model) has recently arrived to the country and I think we will be
seeing more of them used by mothers with babies that cannot take the breast.

There is one private clinic here in Bariloche that has a double electric
pump, although I do not know which type. Our group has a PIS that we lend
or rent (depending on the mother's ability to pay). I know what Medela says
and I understand what has been said here in Lactnet about PIS and multiple
users, but please bear in mind that we cannot afford to buy a Lactina or
other hospital grade pump and this PIS (a gift from my b-i-l when my
daughter was born in 1996 in the US) has saved many a milk supply,
specially for moms with premature babies in NICU. (Please don't take this
the wrong way, but we will not be offended if somebody has a Lactina to
donate... the public hospital could definitely use one).

I think that a pump can be a life saver in stressful situations. I also
think that some mothers do better with a pump than with hand extraction,
although I am convinced that hand extraction is a viable alternative and
have seen proof of this. On a personal note, I have used both the PIS and
hand extraction, not to establish milk supply but with an older baby and
only to extract milk during work hours. Did not see any difference in what
I could extract with both methods and ended up using hand extraction every day.
