Hi,
A pregnant client who has hidradenitis suppurativa was recently referred to
me. I hadn't heard of this syndrome before, and am wondering if anyone has
dealt with a patient who has this and is breastfeeding.
From what I've found so far, there seems to be conflicting thought on
whether this directly involves apocrine glands and also if mammary gland
tissue is apocrine/merocrine. So would she possibly have infected mammary
tissue and ooze pus in her milk?
The other issue is that if a patient is oozing pus around their breasts and
axilla, does this pose a problem to the baby? (HS tends to affect these
areas a lot).
She's had many forms of treatment, and at this point, the only thing that is
effective is excisions of the infected tissue. Because of this, she's
actually had her breasts resected twice already, with nipples
removed/replaced each time, so that she has many challenges ahead.
But I'm hoping to learn from any of you who have encountered this before.
Thanks,
Susan Lawrence, RN, IBCLC, LLLL
