I spoke with the mom with overactive MER / over-supply today.  At last report (last Tuesday) baby was much less fussy.  Today the mom called me in tears.  The baby is back to being fussy most of the day.  She saw the pediatrician on Monday and baby had not gained any weight in 2 weeks.  The pediatrician again advised the mom to put baby on formula because "we've done all of the tests and nothing is wrong with the baby so something must be wrong with your milk".   

Just to recap, baby is 4 months and has been fussy (colic?) since about 4 weeks.  Baby is gassy, fussy, frequent explosive bowel movements, reflux (taking zantac) frequent feeds but never really acts satisfied in spite of excellent weight gain, poor sleeper, arches back and cries at breast.  Mom had been feeding from one breast per feed since the early weeks.  Almost 2 weeks ago mom began using the same breast for 2 feeds in a row.  Baby was better for about 5 days but is now back to the same behavior.  I suggested to mom that maybe baby is unhappy with slower flow but I don't think that is the whole problem.  I'm not sure where else to go from here. I suspect a sensitivity to something in the moms diet and I have asked the mom to keep a 24 hour diary of everything (feeds, sleep, diapers, behavior, what she eats, etc).  I have also asked her to include the tests that the dr. has had done on the baby.  I hope I can find some answers for this mom.  I am wondering now if 24 hours is long enough.  Would 2-3 days or longer be better?  I will post more details as soon as I get them from the mom.
