it may be difficult to find out what this is. i recently went thru this (not 
lactating) and ultimately my large engorged looking breast full of veins was 
a breast abscess that had to be surgically drained.
ultrasound, CT scan, MRI are sometimes used. in my case, none were 
conclusive. finally the breast that just looked engorged got very red and 
you could see it was infected.

she may have to have a biopsy too to rule out inflammatory cancer. if there 
is a breast center where they do breast cancer screening, that would be good 
place to go.
