When Kay Hoover and I were hospital LCs were had a rough-and-ready system of
keeping track of what we did with a bf mother and baby.  There were four
categories, and each higher level of complexity encompassed the simpler
levels.

They were
T   for talk or teach
O  for observe
A  for assist
P  for problem-solve.

The "A" level was where the "hands-on" began.  If we were talking or
observing and believed the mom was having some kind of problem, then we
washed our hands and worked with her.  So "T" and "O" were like our screening
method---but at the same time we could give lots of reassurance and get lots
of information.

"A" covered the kind of latch-on problems that you can "fix" with some work
on positioning---teach the dad how to help, change the pillow placement, show
the mom where to put her hands, teach how to wake the baby up...etc.
Anything that took phone calls to docs or the nursery (about maternal
medications, for instance), repeat visits, obligatory follow-up because they
weren"t "there" yet with their nursing...was a "P."  Teaching a mom to pump
because of a non-nursing baby would automatically be a "P."

The system "just growed" but it served us well and we never looked for
another.  Of course...the *program* didn't last!  So maybe you need something
that looks more official!
