  Women with some retained fragments of placenta can present with "not enough milk" & a slight to heavy lochia. I always ask them about their delivery including delivery of placenta & membranes. I have had a couple of women who had this problem until they were finally referred for scan & they then had an ERPC (evacuation of retained products of conception) after which they lactated beautifully.
