The Ameda One hand pump is a good, simple, manual pump. Your mom is pumping
out a lot of milk, probably a double amount on each breast. What I have seen
is most moms can remove  2-3 oz total, two sides together, on a manual pump.
She might be having this transient pain upon the milk ejection reflex or she
could be having mastitis starting up (because of the fever). Many mothers
with over supply have trouble with plugged ducts and/or mastitis and many
complain about MER pain. Also with a manual pump if the mom really squeezes
it hard, it creates a very high suction. Tell this mom to put her hand all
the way up on the handle to the cross bar and gently squeeze it and release
it. Don't hold it for more than a few seconds. And don't pop that handle up
and down. The most effective way to use a manual pump is to use it 5 minutes
on each side, rotating from breast to breast 2-3 times until the milk flow
has stopped. (Meaning 5 minutes on the left, five on the right, then five on
the left again, five on the right again, etc.) And she should be using it 8
times in 24 hours to keep up her milk supply.
