Seems that it will take some investigative work  to get to the bottom of
this. Handwashing of course is an issue, scrupulously clean pump parts
(where/how do the mothers clean their kits?), sterilizing every day, how
clean is the rental pump or shared nicu pump? what is the protocol for
cleaning it and is it adequate? What are the storage facilities? Is the
refrigerator or freezer in good working order? What about mixing the HMF
which I am assuming is used on the VLBW infants? How/where is it mixed and
under what conditions? Definitely get the infection control experts
involved. Surfaces and people may have to be cultured depending on the
offending organism(s) and their likely hiding places. What do all the bf
VLBW babies have in common? What type of apparatus in being used to deliver
feedings? Are continuous feedings used and under what conditions? I am sure
there are many more questions to be asked.

Remember, if the blood supply were tainted, the hospital would not just do
away with the Blood Bank but would investigate THOROUGHLY to get to the root
of it. Good luck.
