Responding to the question of whether other Children's Hospitals document mother medications... Childrens's Hospital of Pittsburgh, PA, USA.  NO! that would be a NIGHTMARE! Who came up with that idea, the pharmacist?  If there is a medication of concern, most of the docs will order a lactation consult.  I will then pull resources together and provide the info to mom and docs, while also providing the resources so that hopefully they will begin to look for those resources.  Unfortunately, I am the only dedicated FTE to Lactation in the entire hospital so, I see patients by consult only basis.  We also have dietitians that will usually be present periodically through NICU, CICU, transplant rounds or planning meetings.  It is not the most ideal situation but where I practice, documenting every medication that mother takes would lead to many babies with no mothers milk.
