I have been working with a mom who has a now 1 month old son and still
very sore nipples, I have searched the archives and didn't find much.

A little history:
Mom is a g4 p 2 , has lost 2 daughters shortly after birth to a very
rare muscle disorder.
Nursed her first son for over a year and remembers sore nipples but
nothing like this.

I have observer her nurse in the hospital after her was born and when he
was about a week old, good positioning and latch, except for at 1 week
occasional turning upper lip under but mom says this has corrected now.
Mom uses different positions for each feeding, has tried expressed milk,
lanolin and Greers Goo = (zinc oxide, hydrocortisone and nystatin) on
the nipples. The latter was not recommended by me but mom felt minimal
improvement after a few treatments of this.

No tongue tie, digital exam was normal, no yeast seen on mom or baby,
although mom does report stabbing shooting pains in breast after
nursings, and occasional really sore hurting nipples from stimulation
like exposure to cold, denies blanching of the nipples.

Before delivery mom had super itchy breasts/nipples  I spoke to her on
the phone at the direction of her OB who had prescribed the Greers Goo
for her, I suggested she visit a dermatologist at that time, before
delivery, but she didn't.

I spoke with her today at length, she wondered about tx for yeast with
out any visible signs on her nipples or the babies mouth. I told her she
could try this and see if it improved after a weeks trial.  She tried
the gyne lotrimun for about 3 days and thought it was worse so she
stopped.

At this point she hs decided to try hydrocortisone several times a day
for a week and if that doesn't help, try the yeast tx.

Mom really want s to nurse, she is so thankful to have a little one to
nurse after her last 2 major losses that she will endure almost
anything, I want to help her, but am not sure what else to do.

I am at a loss at what else to check or look for........ any thoughts or
suggestions would be most appreciated.
