I would remind mother that every pregnancy is different.  Your body makes new milk ducts for each new baby.  My sister had some hormonal issues after her first was born (totally natural birth) and her milk didn't come in.  She had her second child almost four years later and nursed successfully for almost a year (had to return to work).  So I would say that just b/c she had difficulty with the first it doesn't automatically mean she's going to have difficulty again.  Certainly prepare her with tips for establishing a good supply etc. but I would also provide some encouragement that since this is a different pregnancy, different baby, etc. things could go very differently.  My sister didn't experience any breast change in her first pregnancy and very little in her second pregnancy.
