I have a mom with that too -- third baby, excruciating pain  with all three 
of them.   Nothing could be found -- no infection, no  nothing.  Went to a 
derm -- she couldn't find anything.  No tt,  posterior or otherwise (though 
I didn't see her w/ first two babies) she said  there was not and the babies 
were checked for it.  First two babies also  had CST with no results in 
terms of nipple pain.  Mom denies sexual abuse  of any kind.  No reynaud's.  No 
hx yeast for mom or baby.  Good  asymmetric latch on this baby.
 
Just seems quite odd (to say the least) that this has happened  with all 
three babies.  And yes, she can't tolerate shower water beating  down on them. 
 They are always tender during lactation -- but not during  pregnancy, 
periods or lovemaking.  And she tans, she doesn't burn.  
 
Pumping hurts too, even on minimum and with olive oil  otherwise she would 
pump and feed EBM.

I'd love to see what happens in the  baby's mouth on ultrasound before 
blaming it all on maternal  hypersensitivity. We've seen all kinds of 
distortions in our study,  including a baby who bent the mom's nipple to 
a 90 degree angle during  sucking! The big red flag here is that there is 
more pain on the initial  latch, which usually means the grasp is not 
quite right, even if it looks  okay from the outside.

The other thing I'd look at is if there is a  chronic low-grade infection 
of the nipple that could be ramping up the  pain.
