So....I had an idea to use half of an 8.5x11 page bf care plan (and turn it
to the vertical position) to put the bf basics right on the nurses
clipboards. I also thought about putting the 10 steps to BFHI on the back
(we could write in parentheses what step we are addressing too). Here is
what I came up with and I hope you guys like this. Let me know, I am working
Wed and Thurs this week. Laurie

BREASTFEEDING SUPPORT SERVICES
         CARE PLAN

Pt/family goals:
for example this could say "bf exclusively; or bf for 1 yr; or bf
exclusively at least until ret. to work at 6 wks."

Pertinent findings on assessment:
Mother - this would be if nipples are inverted or mom had br. surgery
or it could say "ample colostrum" or "colostrum easily expressed".
Baby - LGA, IDM or whatever

Care plan (Draw a line through, initial, and date any changes):
1. freq. unrestricted bf and rooming-in. (BFHI # 7,8)
2. if MD orders suppl. this is to be given by FF or SNS.(BFHI #6,9)
3. f/u w/ LC by phone and schedule appt prn; referral to LLL for ongoing
support (BFHI #10)

Carefully document any variance from this care plan
in the medical record and inform the LC of same.
