Gee, Jan, you do realize, don't you, that you've just described nurses'
positions within the hospital structure?  All the responsibility for low
pay, no autonomy, and no respect.  Kinda describes being a parent, too.  My
oldest daughter said it all when she said, "Why would anyone want to be a
nurse when you don't make any money, you have to do everything, and nobody
respects you?"  Out of the mouths of babes (although she's 21 years old!)
And I don't believe that it's coincidence that most hospital-based LCs who
are doing this are also RNs.  Old habits die hard.  We should all be
contracting with hospitals for our services (just like the docs do), as
both LCs and as RNs, but I'm not sure in whose lifetime this will occur.
