As Jane Kershaw suggests, don't rule out psoriasis.
As regards nipple eczema or dermatitis, I have had two cases where this was 
caused by using a swimming pool and leaving the swimsuit on for a while 
afterwards. One mother took my advice and used a topical corticosteroid 
(which then required a script). She also changed out of her swimsuit 
immediately and took a shower. The other, who consulted me at a conference 
overseas, decided not to use a corticosteroid, and just to shower 
immediately after emerging from the pool at the conference venue, a spa. I 
saw her a week later at an event in another city and she reported that she 
had follow my advice successfully.

Hi all, My friend has been struggling with nipple eczema for about 6 weeks 
now. She tried to avoid steroid cream but has now resorted to it. The eczema 
started when she was pregnant then went away. Her baby is now 15 months. Mum 
also has outbreaks of psoriasis. This is her 3rd baby. She has had no bother 
before.Any thoughts would be appreciated. I looked up the 'Breastfeeding 
Atlas'. Only one case is mentioned and steroid cream was used successfully 
to treat the eczema.My friend was sent to specialist clinic to check what 
the problem was then and told it was eczema when she was pregnant. Her
