You might also take a parental growth history. Was parent(s)  small as
infant/toddler?

Anecdotal, I know, but this sounds like my first child's growth history.
I was blessed to have a Ped. who suggested adding two nursings a day (to
counteract the distractibility issue at this age). I was not overly
concerned, having heard at every milestone that baby was bigger than I
at this age...from my mother.  Baby confounded the Public Health
Nutritionist for a while, but by age 2 when  still happy and healthy and
on the 5th percentile (just barely), even she conceded that mine was
just a small child. It also took me a LONG time to get the hang of
pumping. just couldn't have a let-down with the pump unless baby was
nursing on the other breast.

Just something to consider... If Mom or Pop were small babies, could be
genetics. If baby is not dehydrated, is developmentally on target, how
much does size really matter?
