Mary, regarding the working mother who is pumping less, you might want to
ask the mother how old her pump is. This pump may be old or has been used
by many women for several babies and on it's last legs. I see that often
with this particular pump which seems to run for years, but ultimately slows
down.

I encourage moms to jump on this problem with everything, all at once. What
I find usually works in this situation is to get as much skin to skin
contact as mom can devise, immediately. Bathe, sleep, carry - whatever.
Also offer to nurse more often when they're together - not just when baby
asks. Find a way to pump more often at work, even if for shorter sessions,
for awhile. Renting a hospital grade pump for a few days might also help.
Go to bed with the baby for the weekend and ask everyone who loves her to
help her out with food, laundry, childcare. Remove as much stress as
possible. If an older child is not happy away from mom, let them play on
the bed with mom and baby. Mom can read while she cuddles and nurses. Eat
well, and especially the comfort foods of her youth! The various teas
commonly used as galactogogues, such as Fenugreek, may help.

I have seen some situations where babies were not unhappy with the smaller
amount of milk mom obtained from pumping at work because they were able to
obtain more when they nursed directly. So, do ask if the baby is complaining
about the amount received by bottle.

Best wishes,
Anne Grider, LLLL, IBCLC
