Yes--this sounds like a bleb.  Needs to be opened.

But it is odd to have that nipple twice the size of the other one.  Would 
try oral antibiotic if entire nipple is tender/swollen. (Keflex or 
dicloxicillin or Clindamycin as for mastitis)
