I recently received a referral from one of our Pediatricians for a mom & baby 
to come see me, who I had not seen after delivery. The baby had lost a 
pound and 1/2 at the 2 wk check up. Mom reported that baby was latching on 
well, nursing every 2-3 hours for 20 minutes, and she had no pain. On the visit 
to me, the first thing I noted was the tight frenulem, as she was crying when 
she got here. I watch her latch on, and from the outside it appeared pretty 
good: wide mouth, but noted that what baby was actually doing was 
swallowing what milk was dripping when mom let-down. So, I concluded that 
she was probably never getting the hind milk, and luckily mom had an 
abundant enough milk supply and good let-down reflex, so it kept her from 
getting dehydrated, but did not give here adequate nutrition for growth. 

Luckily, this Pediatrician is very supportive of a lactation consultants role, and 
will clip tongues if this is the case. He vocied that he felt pretty foolish for not 
noticing the problem himself, but I assured him that as a mother of 5 children 
who all had to have tongues clipped, I am very attuned to looking for that.

A few days after having the frenulectomy, the baby was gaining very well!

My experience is that baby's will nurse very well as soon as that tongue is 
clipped.
