This is from Becky Ahrendsen RN IBCLC who is using husband Jon's LACTNET.

One pregnant mom I recently talked to thought she had inverted nipples and wanted to know how to prepare to BF. Privately, I asked to see them. They stood out from the areola nicely, but her main concern was something that looked like earwax on the ends of them. Apparently they do invert a bit at the very tip where this substance accumulates. This occurred even before she was pregnant. She said her previous Dr. told her to scrape it off. I asked if it hurt to do that, and she said, yes, it made them sore. When I talked to her current Dr., she had not looked at this mom's nipples and the substance on the end of them.
Any experience with this?
