Hi all,

I must admit to confusion with regards medications and mother's milk.
Have heard Tom Hale speak many times and  each time I feel a little less
confused by the concepts.  Do have both of his books and keep them
within arms reach and use them frequently.  I also use the Rochester
Lactation Data Bank if I still have any question.  At the risk of
appearing stupid, I do have a question that has been bothering me for a
few days.

Have a mom pumping for 34 wk (adjusted age) twins who started taking
Buspar 10mg once a day for anxiety.  Even though Dr. Hale's book said it
probably was one of the better meds for anxiety, ( short half life, high
protein binding), nursing staff were concerned because milk levels were
not available.  Called the Lactation Data Bank and was advised that
waiting 4 hrs after taking med before pumping or feeding would probably
be a good plan.  No mention was made about pumping and dumping this
milk.  I kind of surmised that by doing this we were avoiding peak level
times.

The next day, our plan for this mom was changed to pumping and dumping
this milk after 4 hours and then allowing her to save for the rest of
the day.  I guess the nursing staff was concerned about the amount of
medication that was in the milk during the 4 hours after taking the
buspar.

I have read about the concept of amt of med in milk corresponding to amt
in mothers plasma.  What about the milk produced in the breasts during
the peak level times?  As the amt in plasma decreases does the amt in
this milk also decrease?
