There are AAP guidelines, guidelines in every neonatology textbook and 
pediatric nutrition textbook.  Try starting first with the CPQCC Toolkit, Part II at 
www.cpqcc.org. Richard Schanler's chapter in: Walker, Watkins & Duggan: 
Nutrition in Pediatrics: Basic Science and Clinical Applications, BC Decker Inc, 
Ontario, Canada, 2003, is probably the best recent summary.
