if you think about the baby's lips

Medella instructions: The tube would be parallel to the lips going into
the corner of the mouth.
My former supervisor's instructions, the tube would be perpendicular to the
lips, going under the tongue.

I'm really having a hard time visualizing perpendicular and parallel in
regards to the tubing. And I have a particularly hard time seeing how tubing UNDER
the baby's tongue could be effective at all. I simply tell the moms that
they can either put the tube on top of the breast in alignment with the baby's
nose, coming in between the breast and the baby's palate (in the middle of the
mouth), or they can tape the tubing to the underside of the breast, again
coming in the middle of the baby's mouth between the breast and the tongue. Works
well either way, and one isn't trying to think of geometry (which I was never
very good at, obviously). A lot of moms find it easier to tape the tube to
the bottom of the breast rather than the top, particularly when they are working
on getting the baby on in an asymmetrical hold with more of the bottom of the
breast in the mouth rather than the top.

You are right -- it would be lovely to be able to draw diagrams on e-mail!
