On the very real chance that it may not really be milk intolerance at all, I respectfully suggest it might be worth reading

Some of the symptoms you mention seem like those of relative lactose overload. It makes me wonder if she is a mother who has "milk lakes" meaning a lot of milk making tissue with longer or larger ducts. As a result the creamy hindmilk to slow the emptying of the stomach gets delivered erratically, depending on how often she changes breasts.

If the lactase doesn't get a chance to do its job thoroughly in the small intestine but is "rushed through" to the large intestine by oversupply, that could be a clue to the greenish color of the stools. Green is the natural color of many digestive juices. It's quite possible that other active digestive juices may also still be in an active state and so still have the capability of "digesting" part of the intestinal membrane, accounting for the bleeding.

Considering everything else that's been tried, it would be worth it to see if trying this block feeding system strictly for a long enough time might change some of those troubling symptoms in the baby.
