Check out the AAP Task Force on Breastfeeding's: "Breastfeeding Health
Supervision" booklet.

It is set up much as you describe, and is similar to the way the AAP's
Guidelines for Health Supervision III book is organized.

Also, FWIW, the AAP is currently revising the Bright Futures Health
Supervision Manual and is incorporating much of the Health Supervision III
booklet. I'm on the Infancy Expert Panel and have tried to include as much
breastfeeding information (in the visit by visit/important question format)
as I could while still allowing for room for other topics! ;-D That revision
won't be out for probably another year or so, however.
