A friend of mine recently went through two series of rabies shots along
with her entire family, including her three year old who is still
nursing. (They had bats in the belfry, literally--well, their house
anyway.) I asked her what research she did and what she discovered (I
knew she would have done loads of research) and this is what she said,
sent with permission.

It should be fine. Rabies vaccine is given to pregnant women and to
infants, and studies have shown the effect on nursing infants to be
insignificant. Tests have not been done to determine the amount in
milk, but no adverse reactions have been detected in nursing infants.
(That's from the chemical agents in breastmilk tear sheet in Lauwers
and Woessner.)

I can't find information about the immunoglobulin, but that is also
given to infants and pregnant women, so it should also be okay.

In general, any medication that can be safely given to infants is
safe for the infants of breastfeeding mothers. The dose of the
medicine (if any) that they receive in breastmilk is usually far
below the therapeutic dose that they would receive if given the
medication directly.

The rabies vaccine, while unpleasant, has a good track record.
It's quite safe as vaccines go.

My sympathies to the woman. Rabies treatment is no fun. At least
she doesn't have to put the baby through the vaccine series.
