I would ask the insurance co. what form they want you to use.  Most
companies will accept a HCFA 1500 form which one can often buy at an
office supply store like "Staples".  Or, they might accept the lactation
superbill out of California.  I just explained to AetnaUSHealthcare that
I wanted a M. Classic not a Lactina for someone and they approved it and
let the person get it from a rental station that does not have a DME
(durable medical equipment) agreement with them.  The place with the
agreement only rents lactinas.   Small steps!
