Susan,
I attended a conference with Marianne Niefert a few weeks ago and she is firm that breastfeeding promotion should be done at each and every prenatal visit.

In relation to sustaining breastfeeding, it seems like the more I work with mothers, the more I hear them telling me that the length of time they breastfed is a baby issue. If the baby likes it (or demands it) then many often comment that they had not planned or thought of themselves breastfeeding as long but since the baby liked it or demanded it...
..on the other hand, if the baby was difficult, it sometimes ends sooner than they anticipated. Then are the mothers who are going to do it no matter what they have to endure.

Not sure if this is helpful but I'd love to be involved if you decide to do a study.

Patti
