Cindy,
Does the baby's suck tend to improve with milk flow from the supplementer? I
sometimes find that we can prevent pain and improve suckling by having the
baby organize his suck on the finger (finger-feeding) for a couple of minutes
before going to breast.

<<Her breasts were soft, compression stripe abrasions to her nipples. Color
of her milk had changed from golden to white.>>

I would go directly to double electric pump, since this time is so crucial
for milk production. Besides, what is she feeding the baby? Is she getting
enough w/ the hand pump?

<< She had a rough delivery ("urge to push from the onset of labor", vaccuum,
demerol, epidural), no history of infertility or other medical problems or
drugs, no symptoms of retained placenta, breast changes in pregnancy. Suck
assessment revealed a very disorganized suck and I couldn't get anything
approaching a normal suck with finger training with SNS. They had given a
couple of desperation bottles and baby hadn't sealed or bottled well>>

<<I plan to call an OT for advice tomorrow though I'm not sure if we have one
that is BF friendly. I have requested lactnet searches on 'insufficient
milk', 'disorganized suck', and 'hydotherapy'. I know some of you have used
bathing as well as ?craniosacral therapy (can't remember terms to do proper
searches).>>

You can go to the Upledger Institute website (www.upledger.com) and search
for a cranial-sacral therapist in your area. I would definately have mom do
co-bathing (I always tell mom to do it w/o any stress of "I must get the baby
to nurse", but rather to be very relaxed about it, providing *opportunity*
for baby to lick, suckle, nuzzle). I would also encourage co-sleeping and a
sling (w/ her top off and baby skin-to-skin). I posted a couple days ago
about cranial-sacral. This baby would be a prime candidate. I always try to
get mom to imagine how she might feel if she got stuck all scrunched up and
twisted about, etc--you get the idea. Wouldn't she expect she might have a
headache or have a hard time opening her jaw or feel sleepy? I try to present
it w/o guilt (that word again--hard to avoid, esp since these interventions
are rarely needed).
Good luck!
