It amazes me the wide variation I see in first visits for new
babies.  Some peds always tell the mom they want to see baby by 1
week of age, others do if there is a specific concern, but the ones
that amaze me are the ones who say 2 weeks regardless of
circumstances-even if baby is only 36 or 37 weeks gestational age!
These are also the ones who tend to say things like "Supplement with
an ounce after nursing until your milk is in"!!!!  Apparantly this
is easier for these docs than actually assessing babies
individually.  (Sarcasm definitely intended)
Mom/babies who elect to go home after 1 day for vag delivery or 2
days for C/S get a home visit 2 days later by a visiting nurse.  The
nurse assess both mom and baby and does any appropriate teaching.
If mom goes home without baby, she doesn't get the visit.  If a mom
does not have the "combination stay" (early d/c with home visit) and
I have any concern that feedings are not well established or mom may
not be totally taking in the information given I always recommend
they take the baby in for a weight check either with baby's doc or
with us by the end of the first week.  We do not routinely follow
all breastfeeding moms by phone (I still hope to be able to develop
this as part of our services), but when we have a concern,
especially if the Ped isn't following soon or is one we see as not
very supportive, we arrange to call the mom usually in 2 or 3 days
(with the instruction she call us sooner if having problems or
concerns).  We have "caught" some babies that needed medical
evaluation and wonder what would have developed if baby wasn't seen
until 2 weeks!
