I do know that when I researched primirin/estrogen cream, (prescribed to me post/hysterectomy for vaginal atrophy) similar side effects to other hormonal therapy popped up in studies in terms of blood clots, uterine cancer, cardiovascular risks,  etc.  So I wouldn't assume that just because it is topical it is less absorbed. I actually opted not to use it, as the risks seemed to out weigh the benefits, at least for me. (No one has ever died from not enough sex right:-)
