A midwife I  know of  described  the nipples of a mum of 4 as 'dangerous nipples'  fortunately she  knew what she meant, but  rather a  worrying description.  In her case  her nipples seemed to have elongated, she  did manage to  bf comfortably eventually

You are right -- no milk sinuses -- they evolved into the "Sweet Spot" --  the spot where babies have to latch to get a good MER.  

And I've found exactly what you have found -- especially in multips that  have nursed several babies.  Seems as though the breasts droop, but the  "sweet spot" doesn't move.

Observation only, friends -- don't go looking for a reference.
