This email lifted my day!! You probably dont need to hear this but I feel I need to say it. Nancy, and others in the Academy of Breastfeeding Medicine-Thank you for all you do in getting the message to your peers using evidence based studies and tenacity! And thank you to all my lactnet friends who are on the forefront getting the message to media etc. and other breastfeeding supporters for their daily, individual tenacity as well. Each one of us has to keep repeating the message, feel the message and live the message, as we do each day. Never become complacent. "Its" happening through our combined efforts, each in our own small way. And we can come to this list to get our cups refilled with energy to keep the battle-cry strong in support of women providing human milk to all babies. We must never retreat to less than what we have accomplished over these many years (decades...centuries?) of breastfeeding support. Keep it going! And Happy New Year to all around the world!

But still needing the "warmth" that comes from this list!

"Any honest provider should want to be current, and should want to
practice "evidence-based" medicine, and should want to know all the
information about a given subject before carelessly suggesting things"
As a physician there is no way you can know everything about everything you
will see. As breastfeeding is "natural", many physicians don't know what they
don't know. They recommend what they have been taught - usually influenced
heavily by the formula companies materials.

"Even though I know some HCPs might be offended by a
mother brining in references,"

You don't know how much crap parents bring in off the internet for us
(physicians) to "see". There is a tendency to dismiss everything after you have seen
garbage for so long. How do parents know what is good and what is not? For
that matter, how do I decide what is valid and what is not?

"Why is extreme ignorance of breastfeeding tolerated in the
medical profession, and particularly among pediatricians and others who
work with infants?"

Breastfeeding is still considered a lifestyle choice and not a healthcare
issue by many (including the President of the AAP!) It is not as high a priority
as newborn resuscitation, IV fluid management, preventing kernicterus,
managing asthma, etc. when it comes to educating pediatricians. There a a few test
questions on the Pediatric Boards, and many more than previously because of
the hard work of many breastfeeding-supportive and knowledgeable physicians.
There were an unprecedented 4 questions out of 200 on the neonatal boards this
year! Some of us would like to see a specialty called "Breastfeeding Medicine".
The Academy of Breastfeeding Medicine has taken a first step in that
direction with the designation "Fellow of the Academy of Breastfeeding Medicine" =
FABM.

"What are they learning in medical school?"

Very little, but it is improving thanks to physicians like Audrey Naylor,
Joan Meek, Ruth Lawrence, and many, many others. Also, we are addressing
post-graduate education. I did 40 inservices in doctors offices (FP, Peds, OB) of
the last 2 years myself!

"Should I bother writing a letter to the AAP, just as a concerned
mother, asking why the AAP has not come out with a firm statement that
all HCPs who deal with infants need to be properly educated in
breastfeeding physiology and management and here's how to do it?"

That statement already exists - the 1997 AAP Workgroup on Breastfeeding
Policy Statement, which is currently being updated to add new references.
