If you have a compounding pharmacy in your area they may be willing to mix 
for you.  We have a Medicap pharmacy which I believe is a national chain.  I 
have had a couple of patients try this with great success.  Just a warning, it 
was quite expensive so it would be good for the pharmacist and the patient to 
communicate cost before mixing it.
