Sounds like this poor mom has mastitis and has developed an abscess. Get 
her to a breast surgeon who is familiar with lactating breasts, asap! 
Appropriate antibiotics, both systemic and topical should help, and she 
might need some needle aspiration for that exquisitely painful lump. In 
some cases, abscesses need to be incised and drained. Mom can continue 
to pump once the abscess is treated, and may have to work to rebuild her 
milk supply.

Signs of abscess:
exquisite pain
redness or yellowing of the skin above the abscess, sometimes the 
discoloration is arranged in a ring, with a clearer center.
abscessed area is firm on the edges and softer in the center, if mom can 
bear to have someone palpate it.

Immediate treatment is required, to prevent the abscess from growing and 
destroying much of the breast tissue in that breast. And the doctor 
should prescribe some pain relief as well.
