I'm with Dawn on the feedback form, rather than a complaint form.  You  
could base the items about breastfeeding on the Ten Steps to  
Successful Breastfeeding.  Example: My baby stayed in skin contact  
with me until she had latched on and had her first feed;  my baby was  
not given anything but the breast during my stay; my baby was with me  
at all times during my stay; I was advised to feed the baby when she  
showed signs of hunger; staff helped me to find a good position and  
showed me how babies latch within 2 hours of the birth...  all of  
which the women could strongly agree or disagree with.

I would like to encourage anyone planning to use a Likert scale to  
have an even number of options rather than an odd one.
If respondents can choose between one and five, you will end up with a  
lot of threes, because that is in the middle.  If they have to choose  
between one and six, they are forced to take a stand on every  
question: was it a three or a four?  This will give you results with  
more easily discernible trends.
