Trish-

I differentiate between pain & some level of discomfort. What I like to say
is, "Unless someone's been using your nipples like radio dials every day for
the last year, no one has ever spent as much time there, so expect to feel
some tenderness in the early days as you adjust." I don't expect the
discomfort to last much more than 10 days-2 weeks; if it does, there may be
something else going on. I DO explain what to look for in proper latch. If
she can't see latch because of the way she's built, I will sometimes ask the
mother (especially if she's big breasted) to get a helper/member of
household and a mirror to make sure all is well. Mother makes the
assessment, helper helps with mirror, holding infant, pillows, whatever.

Also, with the pumping thing, the danger is that women who pump will think
that more suction is always better, and I counsel strongly against feeling
that kind of pain b/c it's counterproductive and can do damage.

Best,
