As a neonatologist who has used powdered formula as a fortifier for stable, growing preterm infants for over 30 years, I have never seen a case of E. Sakazakii.  We use powder to maximize the use of mothers' own milk.  Liquid must be used 1:1 with mothers' milk.  I have not used the new concentrated liquid fortifier as yet (from formula company).
