I am neither a nurse nor an IBCLC, but my experience in the hospital and lactation world is that hospitals want IBCLCs also to be nurses so that they can also do nurses' duties, either as part of their job description, or in a pinch when needed, even when hospitals have full time lactation support. 

The issue of RN vs IBCLC, and what the RN qualification brings to lactation consulting has been discussed numerous times on lactnet, and is a source of ongoing frustration for IBCLCs who are not nurses. I don't think there is an easy answer for this.
