I am working with a dyad right now where the baby has a particularly wide
tongue. I would not say the baby fits the usual definition of macroglossia
though. The baby's tongue thickness, elevation, extension, lateralization,
etc are pretty much WNL. The oral frenula is not restricted either. Baby
just seems to have a wider/larger than average tongue. He transfer well (wt
gain WNL) but does struggle to initially latch. Mom is in extreme pain upon
latch on and is taking a little break from direct Bf right now. I feel that
baby is compressing the nipple to compensate for the tongue shape/size in
some way. Once mom is a little more healed up we'll be working on getting
this dyad into some more optimal positions, etc.  So for those of you that
have worked with borderline macroglossia cases or infants with tongues such
as described, please feel free to share any tips that were particularly
helpful for such cases. Thank you!
