I would trust the NICU or Peds RD and their calculations as to nutrient  
needs.  The usual limiting nutrient is protein, not calories, so hindmilk  
usually doesn't work alone.  If the infant appears unsettled, perhaps an  
elemental formula or protein-only supplement would be tolerated better.  Of  
course, human milk should always be the foundation of any nutrition.
