Linda-I believe that your clues lie in the above quote. I would like to know
when the mother noticed her milk coming in with her first two babies.  Mom
did not notice fuller breasts in either of the others.

Did mom supplement for similar problems, or by choice?  Previous
supplementation for low weight gain but she did not have the figures and I
could not see how those babies were affixed to the breast.

3.  Check prolactin levels  OB would need to do this.
4.  Placement of lactiferous sinuses?  When I tried to hand express from her
breast, they felt normal to me (1/2 - 3/4" behind nipple.
5.  Suggestion -- continue to pump, at least until you get prolactin levels
back.  After you get prolactin drawn, try Fenugreek & Blessed thistle OR
Reglan (if no hx of depression) if mom willing to do that.  Have encouraged
her to pump/nurse as much as possible.
6.  Continue use of supplementer if you can get baby to take appropriate
amount out of it.  Mom cannot seem to coordinate this.  I am afraid bottles
may be our only choice.
7.  Good luck -- and keep us posted!  I remember one mom who called me, all
excited one morning, "Jan, my milk came "in" at 2:07 am!"  It was day 12.  I
am hoping for this result.
