And then there are us IBCLCs who live in relatively remote northern areas where food is high priced, herbalists and herbal remedies are difficult to find, and shipping from online vendors is costly AND often slow. Topped off with a strong bottle feeding culture, I'm thankful when a client wants to try domperidone along with lots of effective milk removal and with any luck, skin to skin. Usually the client sees bottle feeding as the only viable option for dealing with supply problems. 



Just my two cents.

I would like to suggest that IBCLCs make it a priority to become acquainted with the foods and herbs that support milk supply. I have very few clients ever even consider domperidone anymore (and none take reglan) because we can use food, homeopathy and herbs with excellent outcomes. I have helped mothers wean off domperidone with moringa.  We have an obligation to be familiar with and IMO competent with these nutrients if we are to be considered skilled providers. It is not okay, IMO that mothers need to get their best breastfeeding advice on FB, AFTER they have already seen one or more IBCLCs.
