The idea is mom is lying flat on her back, and we are attempting to elevate
her breasts above her heart (the old "elevate the affected area to reduce
swelling").  The only way to do this effectively (in most women) is to use a
bra.  Of course, it depends on the size of the breasts and the severity of the
engorgement.  If they are small breasts that are so engorged they aren't going
anywhere anyway, then the bra isn't necessary (except for, as Linda said,
comfort).  If she has large, pendulous breasts, then the bra will help.
