If fixing the latch and breast compressions don't help, this baby might
benefit from temporary supplementation at the breast.  Mom can pump,
then feed expressed milk at breast with a syringe and 5 french feeding
tube (or butterfly needle tube with the needle cut off).   My favorite
way to do this is by letting the baby breastfeed until he stops
swallowing, then deliver a small bolus of milk to get him sucking again.
  Parents need to be taught to respect respiratory pauses, and give a
bolus after the respiratory pause when the baby stalls out.  Of course
one should not push infants with cardiorespiratory issues, but it sounds
like this little guy can do it, but has not really learned how to keep
going.  This will hopefully make the feeding much shorter, and allow mom
to pump so she has milk to use in the syringe for the next inefficient
feeding.  Usually one only needs to do this for a few days with a young
baby to give him a little more milk for his efforts and build his
strength before he can do it on his own.
