   Sounds like this baby is sucking only the nipple.  Try getting him to
open very wide, push him into the breast until his nose touches the breast
and make him get deeper onto the tissue behind the nipple.  He needs to milk
the breast not suck the nipple.  I have had this happen several times through
the years and these moms almost never get sore nipples and  the babies have
poor weight gains.  This baby is living off the let down.
