If there is not such thing as breastfeeding medicine, I wonder what I have been doing for the past 13 years!?!

Seriously, I am the first to admit it should not NEED to exist, but for now, it does.

As for the mastitis protocol, I had nothing to do with it, but feel compelled to point out that in fact it says “If symptoms of mastitis are mild and have been present for less than 24 hours, conservative management (effective milk removal and supportive measures) may be sufficient.” Supportive measures are described in an earlier paragraph.

Has this baby been referred to a Physical Therapist specializing in torticollis and plagiocephaly? Has baby had an MRI or other diagnostic studies to rule out something else going on? To me, CST and chiropractic is not sufficient. JMHO.

PTP

I have been working with a mom whose infant had a very significant lip-tie that was affecting her latch and milk transfer. Revision of this tie has been a lifesaver and has totally transformed her breastfeeding abilities. Previously, Baby could not maintain her latch for a feeding and could not drain breasts. Mom had to pump and offer supplemental bottle-feeds to satisfy Baby. Now Baby can drain breasts and is satisfied with just breastfeeding.

This baby also had significant structural issues, with cranial asymmetry, head turning to one side, arching and mild strabismus. She has had one CST appointment and will be receiving more treatment along with additional chiropractic care.

The most unusual finding was that while Baby can lateralize her tongue evenly, from side-to-side, she does so to the opposite side of gum stimulation. When I trace a finger to the right side of her gums, she lateralizes to the left and vice versa.

Has anyone encountered this with another infant and what is the significance?

Subject: Re: ABM mastitis protocol

The whole purpose of ABM is to have educate and support physicians interested in breastfeeding. We have ILCA and many physicians are IBCLC's as well. I don't blame them for NOT having non-MD's on their board.

Jennifer - Regarding your below message, I am not surprised that you noted: "It could not have been more medicalized." Years ago, I wrote on LACTNET that I wished the Academy of Breastfeeding Medicine (ABM) would change its name to use the word SCIENCE instead of MEDICINE. There is no such thing as breastfeeding medicine, as far as I'm concerned...and I am very uncomfortable with the way breastfeeding is being medicalized. One of the former Presidents of ABM was quick to treat mastitis with antibiotics instead of first rooting out the cause of the mastitis and resolving it with better breastfeeding management.
The responses to my posts were scarce and negative. So breastfeeding is medicalized by ABM and its board of physicians; the ABM board does not include IBCLC icons who are not medical doctors.

Subject: ABM mastitis protocol

How unfortunate that the ABM did not think to include TT as a consideration in its newest mastitis protocol. I think I see more mastitis due to ties than anything else other than maternal diet. Just as disturbing for me is the complete lack of concern for the integrity of the gut and global protocols that do not rush to medicate women for mastitis--this is so easily managed w.o meds --I would guess 95% of the time. Nothing in the protocol is reflective of anything I see in my practice. This is such a disappointment. It could not have been more medicalized.

Subject: What should I do - 1 month old barely back to BW

I am very concerned about a 5 week old baby who had bowel surgery soon after birth. BW was 5#11. DOB 6/8. Lowest weight we know of was 5#7oz. Baby left hospital 7/6 after surgery at 6#2oz (1 month) and I saw on the 11th (4 1/2 weeks) - was still at 6#2. Seen again on the 15th - still 6#2oz. (mom is breast and bottle feeding). Baby has a very weak suck/no milk transfer, I wanted to see if she would at least take something - which she did - 1oz of formula. I called the doctor on the 11th with concerns (he did not contact her nor did she go in) then called him again on the 15th - insisted mom take baby to the doctor to be seen. Mom did - doctor said baby was 6#4oz - and he will see in a week. I am so worried about this baby - she does not look good at all.
